Building Capacity for Lifelong Health

Our Approach

At Vital Clinical Exercise Physiology, we believe everyone has the capacity to improve their health. Through personalised, evidence-based exercise, we support people to build functional strength and confidence while improving their long-term health in a safe, clinically guided environment.

Every care journey begins by understanding you. We learn about your health, goals, lifestyle and what matters most to you, then create an individualised programme suited to your needs and abilities, ensuring the right level of challenge while supporting your progress every step of the way.

Whether you’re managing a chronic health condition, recovering from injury or surgery, improving mobility, or building strength for healthy ageing, your programme is shaped around you - not simply your diagnosis. Progress is regularly reviewed so you can train safely and see your improvements.

As an ONERO™ Certified Provider, we also deliver specialised, research-backed exercise programmes for people living with osteoporosis and osteopenia, designed to improve bone health, increase strength and balance, and reduce the risk of falls and fractures.

Above all, we want you to feel listened to and supported in an environment where you can feel confident in your care, trust the process, and leave each session knowing you’re building capacity for lifelong health.

‍Your Care Journey

Step 1: Getting Started

Getting started is simple. You can begin with a referral from your GP or specialist, or you are welcome to self-refer - no referral is required to book an appointment.

Step 2: Initial Assessment & Individualised Planning

A comprehensive assessment is conducted to understand your health, movement and goals. From here, a personalised exercise program is developed around your needs, abilities and lifestyle.

Step 3: Personalised Clinical Exercise

Your individualised exercise program is carefully guided and progressed to support your health needs - whether managing a chronic condition, recovering from injury or surgery, improving bone health, or building strength, balance and mobility.

Step 4: Strength, Mobility & Functional Training

As you progress, your program evolves to build strength, improve mobility and movement confidence, and support you to remain active and capable in everyday life.

  • No referral needed - you can book directly without a referral.

  • If you are referred by your GP under a GP Chronic Condition Management Plan (GPCCMP), you may be eligible for Medicare rebates. Private health rebates may also apply, depending on your level of cover.

  • Onsite Medicare and private health claiming is available. We also support NDIS participants and other referral pathways.

Community-Based Exercise Physiology | Home Visits

Whether you’re managing a health condition, recovering from injury or simply wanting to move and feel better, your program is tailored to your current abilities, goals and lifestyle.

Meet Your Exercise Physiologist

I believe everyone has the capacity to improve their health at every stage of life. My approach begins with understanding the person in front of me and using evidence-based exercise to build long-term health and physical capacity for the things that matter most to them.

Lennah Adams

MClinExPhys | AES | BPhysio

I’m Lennah, an Accredited Exercise Physiologist and Exercise Scientist with experience in hospital and private practice settings in Australia and internationally. I hold a Master’s in Clinical Exercise Physiology and a Bachelor of Exercise and Sport Science from the University of Sydney, and a Bachelor of Physiotherapy from Kenya.

I support people managing chronic health conditions to improve strength, mobility, balance and confidence so they can live active, fulfilling lives. As an ONERO™ Practitioner, I also provide specialised, evidence-based programmes for osteoporosis and osteopenia.

For me, clinical expertise goes hand in hand with genuine care. I believe the best outcomes are achieved when people feel listened to, understood and confident in the plan we create together. Every programme is tailored to the individual because no two people, or their goals, are ever the same.

Outside the clinic, I’m a proud mum of three boys, a self-confessed tennis tragic and an Avalon local of more than 16 years. It’s a privilege to support my community in building capacity for lifelong health.
